Dr. Sydney Coleman of New York City combines the BRAVA system with Breast LipoStructure12

Dr. Sydney Coleman presented his experience with using the BRAVA system before and after fat grafting to the breast at three conferences in the last week:

Dr. Coleman showed examples of breast augmentation and reconstruction using autologous fat combined with the BRAVA system.  He felt that the BRAVA system of reverse tissue expansion has been improving the excellent results he has already been obtaining with fat grafting to the breast.

    I have read through your web site and see the results you have been getting. I am hearing a lot about autologous adipose stem cells being used to enhance fat grafting both manually and with the Celution device by Cytori. What is your opinion of using the stem cells for breast enhancement?

  2. Dr. Coleman:

    Matt,
    I have been doing a good deal of research on the stem cells found in fatty tissue. The Celution device is a promising manner of “super-charging” fat tissue to give it a higher concentration of stem cells. I think they are about to release more studies, which support that this improves the performance of grafted fat.
    However, few years ago, I developed some simple techniques, which concentrate the stem cells in the fat. I use that method to concentrate stem cells when I want a specific effect on the surrounding tissues.
    I now use my techniques for concentrating fat in almost every fat grafting case. Not only does it seem to stabilize the volume of fat, but also it helps with skin problems such as wrinkles, stretch marks or the effects of radiation or sun damage.

    Dr. Coleman,

    My sister and I have been researching fat grafting for breast augmentation, and you definitely have the longest experience and great results. You are obviously the master of fat grafting to the breasts. My older sister has already had breast implants, but they have made her breasts look unnatural (hard and weirdly shaped). I am afraid that silicone breast implants will do the same thing to me.

    I have heard that fat grafting to the breast costs a lot more much more than silicone implants. Why is it so expensive? And finally how much would it cost to have you perform fat grafting to the breast for me and for my sister who has the problem with her implants?

  4. Dr. Coleman:

    The price of breast augmentation with fat depends on how much of an augmentation and reshaping of your breast that you desire. For a simple augmentation the price usually runs from about $20,000 up, depending on the complexity of the procedure. For placing around silicone implants to cover the edges, disguise the edges or to help soften the implant the cost is usually less because the amount of fat that I need to transfer is usually much less.     Dear Doctor:
    Recently I just found out that I have tubular breast.I did a lot researchs and talked to a lot of doctors to see if there is any way that I can have normal breast without implants since it is very scary if the implants go wrong. However,most of the answer is better having a implant correctio.I have only see your 2 of your case picture and I would like to know if you are willing to send me more. By the way, I also want to know the approx. price for tubular breast correction and if it can be covered by the insurance? tks

    Rina,
    The best solution for tubular breast now appears to be structural fat grafting. If you can send us photographs of your breasts, or come by the office for a consultation, I can better advise you on your situation or send you photos of similar patients.

    The professional fee for tubular breast correction varies depending on how much augmentation is performed at the procedure, but can run from $12,000 to $30,000. Insurance companies are variable in their policies, but most do not cover tubular breast correction unless there is a functional problem.

    Dear Dr Coleman,

    I had breast reductions when I was 18, and then reconstruction/saline implants when I was about 22 – this because the reduction was poorly done. I am now 32 and I am having a lot of issues with wrinkling and hardening (esp of my L breast). I wanted to know whether it was possible to have the implants taken out and replaced by fat grafting. Also, whether you have done any such procedures and may have case photos.

    Yes, fat grafting can help not only with the wrinkling, hardening and internal scarring present, but also can restore your volume. It will most likely take two procedures.

    I have been reading about this, and really want to get it done. This procedure seems to outshine breast implants – and having to take our breast implants at max every 10 years really turns me off. I just have a question – I have A cups, and I’ve seen I could only go up to B cups – but using stem cell or adipose, something like that, would be more beneficial to gaining at least a C cup.

    Do you think C cups could go be brought up to C cups? And how long, generally, does this procedure last for? A lifetime?

    Karen,
    You are absolutely correct. This procedure does outshine breast implants. We have been seeing that people who have adequate donor sites and who religiously use the Brava before and after their procedure end up with a much larger correction than our office’s first patients (see my website) did. We feel that pretty much what you see at about 4-6 months after the procedure is close to the end result. Of course, women naturally lose some volume of their breasts with age (especially around menopause).

    Hello Dr. Coleman,

    I want to have this procedure done, but I dont want to have to gain weight and I dont want to have to give up exercising. Will I be able to exercise after having procedure done? Also, you mentioned somewhere about the effect of fat on the surrounding tissue. What does that mean? does the new fat change the consistency of the surrounding tissues? and what happens when the 20% of fat reabsorbs? Is it safe to reabsorb fat? also, I want the breasts to feel soft, but I heard calcifications and fat necrosis can make them hard?

    Leese,
    We do not recommend that patients gain weight before fat grafting, especially to the breast. We think it puts them into an unhealthy state, and creates fat which can be less than optimal. We recommend patients be active even immediately after the fat grafting procedure, and begin moderate exercise (treadmill, stretching, et cetera) after one week. We encourage people to resume normal exercises at about two weeks after fat grafting procedures.

    The important factor is that the 10 to 20% of the fat that goes away does so evenly. That appears to be one of the big strengths of my technique. The resorbed fat is carried away by your white blood cells in a natural way much like it would if you bruised your breast by blunt trauma.

    Breasts after augmentation with fat grafting are usually incredibly soft and natural. It is usually impossible to detect the infiltrated fat even with careful examination. However, it is definitely possible to have calcifications and fat necrosis with fat grafting to the breast, as in every procedure to the breast. Silicone implants are especially prone to calcification and scarring. If fat necrosis occurs in a properly fat grafted breast it is usually isolated to a small area.
