<h3><span style="font-weight: normal;">&#8220;It is the last place in the world you’d expect to hear someone praising fat. But, in a chic Manhattan plastic-surgery clinic populated by impossibly lean women, Dr Sydney Coleman is doing just that, while explaining a new technique with radical implications for cosmetic and reconstructive work.</span></h3>
<h3><span style="font-weight: normal;">LipoStructure, or lipomodelling as it’s also known, involves taking a patient’s own body fat and reinjecting it to sculpt, fill, correct and enhance other areas. We may have got used to the idea of injectable fillers for the face; now it’s body recontouring that’s the growth industry. And, for more and more surgeons, fat is the tool of choice. In the right hands, the results are extraordinary: it can fill scars and hollows, smooth creases, retouch cellulite dimples, as well as plump and sculpt breasts and bottoms.</span></h3>
<h3><span style="font-weight: normal;">One of the most popular procedures Dr Coleman offers is taking fat from the abdomen and reinjecting it into the breasts, which is, as one surgeon puts it, “the answer to a maiden’s prayer” – weight loss, waist definition and breast enhancement all in one procedure.</span></h3>
<h3><span style="font-weight: normal;">But aren’t people squeamish about using their own fat? “They love it,” Dr Coleman says, with a big smile. “The abdomen and love handles are always my first choice because that makes the woman thinner.” Fat can, of course, be taken from anywhere it’s not wanted on the body and, as Dr Coleman says, “There’s no blood and no cuts. There are just these little puncture sites.”</span></h3>
<h3><span style="font-weight: normal;">Coleman first started investigating fat-grafting in the late Eighties when, he explains, “I’d go to a cocktail party and women would pull me aside and complain about their thighs and abdomens after liposuction. So that was the original motivation – to help these women who were coming to me saying, ‘What am I supposed to do now?’?”</span></h3>
<h3><span style="font-weight: normal;">Back when liposuction was a new procedure, many surgeons would remove too much fat from thighs, leaving them hollowed and irregular, he says. “I visited people in California and Virginia and spoke to people in France who were doing fat-grafting for the correction of liposuction deformities, but they’d never had good experiences. So I looked at what they were doing and tried to do it a little differently. I tried to bring it down to more basic principles.”</span></h3>
<h3><span style="font-weight: normal;">The principles may be basic, but the work is also extremely precise. Coleman’s technique involves harvesting fat very gently through low-vacuum cannulas before using a centrifuge to separate the oil and water. (Traditional liposuction techniques, which involve exerting a lot of negative pressure, can harm the fat and increase the chances of it dying once it’s been grafted, meaning it forms hard lumps.) The treated fat is then reinjected, one droplet at a time, in a lattice formation. It’s a delicate and painstaking process, with no room for error.</span></h3>
<h3><span style="font-weight: normal;">The surgery’s “two-for-one” aspect is not its only appeal. Unlike most conventional breast augmentation surgery, there is no scarring. There are also none of the aesthetic and practical problems that come with implants, and fat-grafting affords the surgeon more control – and room for artistry. “With silicone, you’ve got a bag and you make a hole and stuff it in the hole,” says Dr Coleman. “It’s a prefabricated volume. But with fat-grafting, you have to completely visualise everything in three dimensions. You can really shape the breast and improve projection and I’m able to go anywhere – to make cleavage, to feather into the side.” Some of the most impressive before-and-after pictures he shows involve subtle but radical changes to breast shape: an obvious implant, for example, is edged with fat and instantly becomes natural-looking.</span></h3>
<h3><span style="font-weight: normal;">Linda Francipane, a 44-year-old hairdresser from Queens, was an early patient of Dr Coleman’s and underwent breast augmentation surgery having spent “many, many months” researching her options. This included speaking to a lot of strippers about their implants. “I’d say 40 to 50 per cent had some kind of issue with them – whether they leaked, or the look, or the feel,” she says.</span></h3>
<h3><span style="font-weight: normal;">She was nonetheless determined to do something about her flat chest. “You’d be out clubbing,” she says, “and all the hot girls had tremendous boobs and I really had nothing, like nothing. This was the time when everybody was getting implants and I was like a little boy. I just wanted to get in the game.”</span></h3>
<h3><span style="font-weight: normal;">When a friend told her about Dr Coleman’s technique, “It seemed like a fabulous alternative to putting something foreign in my body.” She went from “below an A” to a 36C and says, “The way he did them, you can’t tell. It’s terrific. It changed my life.” She says she’s still receiving compliments.</span></h3>
<h3><span style="font-weight: normal;">Tal, a 36-year-old from Portland, Oregon, is another of Dr Coleman’s patients. She had implants when she was 19 but was so dissatisfied, she had them removed after just nine months. “I couldn’t breathe properly. It was just a very foreign feeling,” she explains.</span></h3>
<h3><span style="font-weight: normal;">As soon as she heard about breast augmentation by fat-grafting, she booked a consultation with Dr Coleman. “I thought nothing could be worse than what I had, so, even if he messed up, it would still be better. I had nerve damage from the breast implants, and stretch marks, and some scarring from under the nipple where it had been cut open. The tissue gets so stretched that I was actually flatter after breast implants.”</span></h3>
<h3><span style="font-weight: normal;">She wanted “breasts that were heavier at the bottom – a kind of Seventies look. I wasn’t comfortable being showy – my identity is really that of a flat-chested girl. It’s hard to create a natural-looking shape; I don’t know how he does it, but it’s just perfect – exactly what I wanted. He’s an artist. I don’t know if I’d trust anyone else.”</span></h3>
<h3><span style="font-weight: normal;">Tal also adds, “I don’t even wear a bra,” before laughing and reflecting, “I probably should, since I made such a large investment.”&#8221;</span></h3>

<h2><span style="font-weight: normal;">Once something of a fringe procedure, fat grafting was pioneered by Coleman in the mid-eighties, when the emergence of liposuction meant that large volumes of fat were suddenly available.  These days, surgeons travel from around the world to learn his trademark LipoStructure technique.  And while pharmaceutical giants are racing to develop synthetic counter-parts for big-picture recontouring—like Macrolane, the hyaluronic acid-based filler that has received mixed reviews in Europe, where it’s approved for use&#8212;plastic surgeons agree that the unique advantages of fat, which is easily recognized by the body and, once grafted, may last anywhere from several years to a lifetime, make it </span></h2>

			<content:encoded><![CDATA[<p style="text-align: center;"><strong>Sydney Coleman of New York City Presents Studies which Validate the use of Fat Grafting to Reverse Radiation Damage</strong></p>
<p>At the annual meeting of the European Association of Plastic Surgeons this year, Sydney Coleman presented cutting edge studies which verified in animal models the healing effect of structural fat grafts on skin that had been exposed to therapeutic radiation.  For over a decade, Dr. Coleman has been <a href="http://www.sydneyreesecolemanmd.com/" target="_blank">treating patients with damaged tissue due to radiation injury</a>.  Not only has he witnessed the survival of the fat grafts, but also he has noted a significant improvement in the quality of skin in almost every patient.</p>
<p>The research was performed in the laboratories at NYU Medical Center.  Dr. Coleman reports, “Our research verifies my observations over the last ten or more years that fat does not just survive when placed into irradiated tissues, but it reverses the radiation damage in almost every case.”</p>
<p>In the NYU study, mice were exposed to enough radiation to cause hair loss (alopecia) and skin damage over a four-week period.  At four weeks, fat grafts that Dr. Coleman personally processed using the &#8220;Coleman technique&#8221; were transplanted under the radiation damaged skin.  Other mice were treated similarly using normal saline (saltwater).   In the mice treated with fat grafts, first the blood supply increased and then the hair grew back, the scarring reversed and the skin softened.  In the mice treated with normal saline, the hair loss and scarring progressed and the mice never healed.”</p>
<p>Dr. Coleman further comments, “these findings verify that this type of fat grafting alleviates radiation damage to skin and underlying tissues by improving the blood supply and softening scarring.”  This process is probably due at least in part to <a title="Stem cells" href="http://www.drscoleman.net/2008/08/stem-cells-and-breast-surgery/" target="_blank">stem cells</a> and hormones present in structural fat grafts. “The best way to think of these primitive cells present in fat grafts is as ‘repair cells’ that actively maintain your body by repairing damage as it occurs daily.&#8221;  (If you scratch your skin, or break a bone, et cetera).  &#8221;When placed into an area of subacute damage such as a radiation injury, the ‘repair cells’ and hormones heal the tissues.  This is truly an example of using a person’s own body to heal themselves…regenerative medicine.”</p>
<p>Surgeons are hesitant to operate on body areas which have been treated with radiation because they heal poorly or not at all after a surgical procedure.  Recent evidence points to the healing of radiation injury and <a href="http://medical.preferredconsumer.com/cosmetic_surgery/aesthetic_trends/lipostructure_fat_grafting.html" target="_blank">even improvement in the appearance of aging skin</a> by fat grafting to an area.  The healing most likely takes place <a href="http://www.sydneyreesecolemanmd.com/" target="_blank">by bringing in stem cells</a> (or repair cells) which build new blood vessels and capillaries inthe irradiated skin, muscle and bone.</p>
<p>Dr. Coleman and several plastic surgeons in Europe, have been using his fat grafting technique to cure radiation damage in many breast cancer patients.  An extraordinary <a title="Jaw deformity" href="http://www.drscoleman.net/2009/07/facial-reconstruction-new-york-city/" target="_blank">reconstruction of a jaw deformity from therapeutic radiation</a> was mentioned in this blog last year.  Click <a title="Radiation Damage Treated with Fat Grafting" href="http://www.drscoleman.net/2009/07/facial-reconstruction-new-york-city/">here</a> to read the posting.  Dr. Coleman and other plastic surgeons in Europe have had success in reversing alopecia (hair loss) and other scalp damage from therapeutic radiation.</p>

<p>My office has received many inquires about the cost and the procedure of fat grafting to the breast.  I have therefore written up the following description of my consultation, some important points of my procedure and some explanations of the cost.</p>
<p><strong>Planning Consultation: </strong>The most important first step for fat grafting to the breast is for you to help me design a procedure that will give you the shape and size that you desire for your breasts.  I do that by first examining and photographing your body. Then I study the photographs with you. Together we design an individualized plan to change both your breasts and to enhance your body with the harvesting of fat.  I draw out blueprints of your unique procedure using pencil tracings that depict your specific body and the changes you seek.  I like to color code the blueprint to indicate where I plan to remove the fat, where you desire specific changes in size or shape, and where the tiny entrance sites will be placed (see below).  I use the same sheet to make notations as I review your informed consent.</p>

<p><strong>Special Considerations: </strong>The decision to use the <a title="Coleman using the BRAVA System in Breast Augmentation" href="http://www.drscoleman.net/2009/09/dr-sydney-coleman-of-new-york-city-combines-the-brava-system-with-breast-lipostructure/">BRAVA system</a> should be made weeks before the procedure.  Please refer to my last post for comments.</p>
<p><strong>Marking: </strong>On the day of the procedure, I follow the blueprint to mark your body with a color-coded system (see below).  This lets you understand from where I am removing fatty tissue and how I am placing it into your breast to improve your shape and increase your breast size.</p>

<p><strong>Harvesting &amp; Refinement: </strong>The procedure for breast augmentation with fat involves first harvesting fat from the body.  Some women have enough fat in one location (outer thighs or lovehandles), which makes removal much simpler.  However, many women have had liposuction to their outer thighs and lovehandles, or simply do not have enough fat in one area.  In those women, I consider numerous areas for harvesting (abdomen, flanks, inner thighs, knees, calves, arms, et cetera). Obviously, this is an important part of the procedure, and you and I will want to design the removal so that your body shape is enhanced and problems are avoided.  Because of the amount of fat tissue that needs to be removed, I usually perform the procedure under general anesthesia.  However, for much smaller procedures, I use local anesthesia with sedation.</p>
<p><strong>Grading the fat for selective placement: </strong>After the fat is harvested, I refine and concentrate the fat tissue using a centrifuge to remove any non-living components (oil, water, blood, et cetera).  My experience has taught me to grade the fat into different qualities, depending on density and other factors. I use the graded densities to attain different effects. This is one of the keys to successful fat grafting.</p>
<p><strong>Sculpting while layering in the graded fat: </strong>I use the concentrated fat tissue to meticulously sculpt the breast.  The technique I developed alters the shape and size of the breast by placing miniscule amounts in thousands of passes through three or four tiny entrance sites located in the lower fold of the breast and the areola.  The sculpting requires a three dimensional vision that must happen while the fat is being placed into the breast.  Many surgeons place fat then try to move it around.</p>
<p><strong>Price: </strong>How is the price determined?  First of all, price varies depending on the difficulty of removing fat from select parts of your body, processing and concentrating the fat.  If it is going to take me one hour to harvest the fat, then I am able to charge much less than if it will take me two or three hours.  Then the difficulty of the meticulous placement into the breast needs to be considered.  Previous scarring from implants or surgery will make the procedure more difficult.  The more fat I harvest and place, the more that I need to charge.  Simple, uncomplicated augmentations start at $20,000.  <a title="Breast Implant problems treated with fat grafting" href="http://www.drscoleman.net/2008/06/breast-implant-problems-solved-with-coleman-fat-grafting/">Treating the problems of breast implants with fat grafting</a> is usually less depending on the amount of fat needed to be placed and the amount of scarring present.  <a title="Breast reconstruction with grafted fat" href="http://www.drscoleman.net/2008/10/coleman-lectures-on-fat-grafting-for-breast-reconstruction-at-the-american-college-of-surgeons/">Reconstructive procedures</a> after breast cancer or from birth defects can often be covered at least in part by insurance.</p>

<p>Dr. Coleman showed examples of breast augmentation and reconstruction using autologous fat combined with the BRAVA system.  He felt that the BRAVA system of reverse tissue expansion has been improving the excellent results he has already been obtaining with fat grafting to the breast.</p>

<p>The patient in these photos presented for an enhancement in the shape of her breast, and an improvement in the size of her lower breasts. Before and after about 200 ml of refined fat into each breast.  She wore the BRAVA before the procedure, but not afterward.</p>

<p class="MsoNormal"><span>In a new statement by ASPS entitled <a href="http://www.plasticsurgery.org/Medical_Professionals/Health_Policy_and_Advocacy/Health_Policy_Resources/Policy_Statements__Guiding_Principles.html"><span>Fat Transfer/Fat Graft and Fat Injection ASPS Guiding Principles</span></a>, fat <a title="Fat Grafting to the Breast: description" href="http://www.lipostructure.com/breastaug.html" target="_blank">grafting to the breast</a> has been cautiously approved by the ASPS for use in breast reconstruction, breast shaping and breast augmentation.</span></p>

<p>After ten years of experience with fat grafting to the breast using his specific technique, Dr. Sydney Coleman began presenting his results at plastic surgery meetings in 2005.  Perhaps the most pivotal presentation by Dr. Coleman was in 2006 at the American Association of Plastic Surgery.  His impressive long-term results resulted in an active discussion in the Plastic Surgery world on the use of fat grafting in the breast.</p>
<p class="MsoNormal"><span>The American Society of Plastic Surgery had previously directed plastic surgeons to avoid placing fat in the breast due to safety and outcome issues.  Dr. Coleman and others demonstrated that the original concerns were not based in any scientific evidence, and instead the evidence was in support of fat grafting to the breast. </span></p>
<p class="MsoNormal"><span>As a result of Dr. Coleman’s many presentations, he was able to persuade Roxanne Guy, then president of the American Society of Plastic Surgery, to form a task force of plastic surgeons to revisit the issue of fat grafting to the breast.  The task force began meeting in September 2007.  “The task force reviewed the scientific literature, critically appraised the information available, and developed <strong><em>evidence based</em></strong> practice recommendations.  The findings of the task force were then submitted to an approved without significant change by the American Society of Plastic Surgery and the Plastic Surgery Educational Foundation executive committees.</span></p>
<p class="MsoNormal"><span>The recommendations indicated that “<strong><em>Fat Grafting may be considered for breast augmentation and correction of defects associated with medical conditions and previous breast surgeries.” </em></strong> The society also indicated that people should proceed cautiously because “<em><strong>results are dependent on technique and surgical expertise”</strong></em> as in every other surgical procedure.</span></p>

<p class="MsoNormal"><span><a href="http://www.drscoleman.net/wp-content/uploads/2009/02/kathy-markings-pixilated-nipples.jpg"></a>THE DOCTORS</span> television show followed a patient of <a href="http://www.lipostructure.com/"><span>Dr. Sydney Coleman&#8217;s</span></a> through her breast reconstruction after breast cancer. She had already had a lumpectomy, radiation therapy and multiple reconstructive procedures. Dr. Coleman reconstructed not only the hard, scarred area of the lumpectomy but also placed fat around her hard implants in an effort to soften them. He also treated the opposite breast that had been left with an abnormal shape after a breast lift.</p>
<p class="MsoNormal"><span><a href="http://www.thedoctorstv.com/main/show_synopsis/100?section=synopsis">In the segment, Dr. Coleman explains how LipoStructure differs from liposuction and then he simulates the steps of the procedure live on the show. </a><span> </span>Liposuction is the procedure in which fat is removed from beneath the skin through hollow tube connected to a source of suctioning.<span> </span>On the other hand LipoStructure is a type of fat grafting procedure in which the fat tissue is removed from one area and placed back into an area in which there is something missing.<span> </span></span></p>
<p class="MsoNormal"><span>LipoStructure is a trademarked word, which describes the procedure Dr. Coleman developed.<span> </span></span></p>
<p class="MsoNormal"><span>The first step of LipoStructure is to harvest the fat using a syringe. On the set of The Doctors, Dr. Coleman demonstrated the first step of LipoStructure by placing a blunt needle connected to a syringe into simulated fat.<span> </span>Then by placing a gentle suction on the syringe with his hand, Dr. Coleman was able to suction the simulated fat up into the syringe.</span></p>
<p class="MsoNormal"><span>Dr. Coleman then demonstrated the placement of fatty tissue using a clear plastic model.<span> </span>Through one entrance site in the plastic mold, the audience can clearly see Dr. Coleman move the blunt needle back and forth fanning over a large area to demonstrate the method in which he normally places fat into the human body.<span> </span></span></p>
<p class="MsoNormal"><span>After the demonstration of fat grafting, The Doctors reenacted the life of one patient “Kathy” from the time she discovered her breast cancer through multiple cancer and reconstructive surgeries that she said left her horribly scarred and disfigured.<span> </span>After extensive research she was able to find Dr. Coleman.<span> </span>The Doctors television show filmed <a title="Kathy's surgery" href="http://thedoctorstv.com/main/procedure_list/35">Kathy’s preparation for surgery and then the entire surgery by Dr. Coleman</a> in New York City.<span> </span></span></p>

<p class="MsoNormal"><span>Toward the end of the television segment, Kathy sat in the procedure room two weeks after her operation and updated Dr. Ordon and Dr. Coleman on her recovery.<span> </span>She said that in a short amount of time she was able to perform routine chores, go shopping and lift her grandchildren.<span> </span>She announced that her scars had faded dramatically and she feels great.<span> </span><span> </span></span></p>
<p class="MsoNormal"><span>Dr. Coleman’s LipoStructure technique not only changes the shape and size of your breast, but it <a href="http://www.lipostructure.com/face.html">can take years off your face</a>! On the show, Dr. Travis shows before-and-after photos of another patient. Nearly four years after her surgery, she still retains an amazingly youthful appearance.
