<p><em><strong>While fat grafting to the female breast has been growing in popularity, chest enhancement with fat grafting is available to men as well.</strong></em></p>
<p>Before Dr. Coleman ever placed fat into the breast of women in 1995, he performed successful fat transplants to the male chest to enhance shape and size of the pectoralis muscle.  These procedures were usually performed on men who were already muscular through weight training and diet, but had reached a plateau.  Not only can fat grafting to the pectoral muscle increase chest size, it can also improve definition of the inner and lower lines of the chest.</p>
<p>The donor fat is usually harvested from the patient’s lovehandles, flanks or abdomen and is transplanted to the chest muscles. Most people can return to work in a few days, and return to their gym routine between one and two weeks after surgery.  The patient will like the way they look a few days after the procedure, the bruising and swelling takes months to completely resolve.</p>

<h2><span style="font-weight: normal; font-size: 20px;">&#8220;In a nutshell, here’s how it works:  Fat is suctioned from the abdomen, love handles, or back—&#8217;anywhere there’s excess,&#8217; says Coleman—using low-pressure liposuction, and is transferred to another area of the body through a series of precise injections with a syringe-like instrument.  There, the tiny blood vessels within the fat will connect to the tissue around it and take up residence in their new environment.  Once it settles into place, you may see more than an increase in volume.  &#8217;The quality of the skin—its color, its texture—improves,&#8217; says Coleman, who also notes a softening in the appearance of stretch marks and cellulite.  That’s because fat is loaded with adult stem cells—as many as one million per cubic centimeter, by recent estimates—making it &#8216;a dynamic, living form of repair and rejuvenation.&#8217;”</span></h2>

<p>Whether forearms are small from birth defects, have missing areas as result of scarring from an accident, or have not grown enough despite a bodybuilder&#8217;s efforts, there are alternatives.  One of the alternatives is structural fat grafting. </p>
<p> </p>
<p>A 36 year old amateur bodybuilder presented frustrated because his forearms had not developed as well as the rest of his body after years of trying. </p>
<p>For this patient a thin layer was placed under the skin over a large portion of the forearm and back of the hand.  In addition, some of the fat was used to sculpt the appearance of the forearm by filling in areas which the patient felt were especially deficient.  A total of 201 ml of refined fat was placed in the right forearm and 192 ml on the left.  In addition 29 ml was placed over the back of each hand (30 ml is an ounce).  For safety reasons, no fat was infiltrated deeply into the muscles or around the muscles.The patient returned 20 months after the procedure, thrilled with the results, but desiring more volume still. He has obviously continued to exercise, but he feels that most of the change is from the fat grafting.  Of particular note, he is surprisingly more vascular in appearance despite the superficial placement of the fat.</p>

<p><em>&#8220;Dr. Sydney Coleman, a plastic surgeon in New York, has developed a procedure over the last 10 years that involves putting up to a thousand tiny injections of a person&#8217;s own fat into their hands; the procedure, which costs at least $12,000, is done in-office and the results last over a decade, he says. The patient is likely to endure about three weeks of puffiness and redness, he said.</em>&#8221;</p>

<p>Grafted fat does more than simply restore fullness.  After placement of the fat, changes occur in skin overlying the grafted sites over the next few week, months and years (for instance, immediately after implantation, in the first months, at one year, three years, five years, eight years et cetera).  The volume of the fat appears to stabilize four months after the procedure, but the texture of sun damaged or scarred skin appears to improve in the ensuing months and years.</p>

<p>A simple technique of injecting fat under the skin of an aging hand can achieve long-lasting rejuvenation. This specific technique was first described by Sydney Coleman <a href="../../../../../sydney-r-coleman-curriculum-vitae/sydney-coleman-lectures-panels/">in 1996</a>.  He eventually published the technique in 2002 in the journal <em><a href="http://www.plasreconsurg.com/pt/re/prs/abstract.00006534-200212000-00017.htm;jsessionid=L92G3yjtytcJGF67znSQ1G7zKrwJLst1C5DTLk2MMMBp2PBgbXL2%21-711215224%21181195629%218091%21-1?index=1&amp;database=ppvovft&amp;results=1&amp;count=10&amp;searchid=2&amp;nav=search">Plastic and Reconstructive Surgery</a> </em>with an extensive discussion in his book <em><a href="http://www.qmp.com/product_dtl.php?QsType=B&amp;QnProductID=19&amp;QnParentSpecialtyID=6&amp;QnSpecialtyID=18&amp;bSubspecialty=Y">Structural Fat Grafting</a></em>.  Aging of the hand occurs in several ways, one of them is a subtle loss of fullness under the skin (atrophy) and another involves deterioration of the skin itself.  <a href="http://www.lipostructure.com" target="_blank">Coleman fat grafting</a> appears to affect both of these problems.</p>
<p><em><strong>How does a hand age?</strong></em></p>

<p>Over the back of an aging hand, fullness gradually disappears allowing the veins to become prominent and their blue color deepens.  The white color and anatomic details of the tendons in the back of the hand become more visible through the thinning skin.  In addition, the skin looses elasticity assuming a texture more like crêpe paper and wrinkling becomes more prevalent. The spaces between the bones of the hand deepen, especially between the thumb and the index finger; and this wasting can be further complicated by loss of muscle volume with aging or disease.  This loss of fullness of the hand and fingers makes the joints appear enlarged and arthritic.</p>
<p><em><strong>How does injected fat rejuvenate an aging hand?</strong></em></p>
<p>Fat injected in a specific structured fashion to position many tiny parcels of fat over the dorsal hand creates a system of support as well as an expansion of the skin. The physical presence of a thin layer of tissue will not only decrease the blue of the dorsal hand veins and the white of the extensor tendons but will also soften the shape of veins and tendons. The overall effect is to create an apparent thickening of the aging skin of the dorsal hand with an underlying youthful-appearing subcutaneous fullness. However, a second method of rejuvenation has just recently become apparent.  Fat has the highest concentration of repair (stem) cells of any tissue in the body.  Placing fat full of <a href="http://www.drscoleman.net/category/breast-lipostructure/" target="_blank">stem cells</a> under the skin of the hand where there is usually little fat may have a repair effect on the skin.  Such a repair effect can reverse the damage to the skin caused by aging and sun exposure.
