Lara,
Fat grafting is one of the best ways to soften and enlarge a tip such as you describe.

Dear doctor!
I had several revision rhinoplasty.
I have rib cartilage in my tip and it's hard when you touch it.
I would like to have bigger nose tip- for 1mm. Is it possible for fat to survive in my tip even there is rib cartilage and it's quite hard?
Thank you

Rita,
Yes, cryolipolysis could be used to decrease isolated areas of fat transplantation. The current limitation on using the Zeltiq device for that purpose are the applicators themselves. The applicators cover a relatively large area, and future ones will cover a smaller area for targeting smaller bulges.

Leese,
We do not recommend that patients gain weight before fat grafting, especially to the breast. We think it puts them into an unhealthy state, and creates fat which can be less than optimal. We recommend patients be active even immediately after the fat grafting procedure, and begin moderate exercise (treadmill, stretching, et cetera) after one week. We encourage people to resume normal exercises at about two weeks after fat grafting procedures.

Please see the <a href="http://www.drscoleman.net/2010/07/sydney-coleman-interviewed-for-stem-cell-facelift-using-structural-fat-grafting/" rel="nofollow">post on the “stem cell effect”</a> to help you understand the improvement in the quality of your skin after fat grafting.

The important factor is that the 10 to 20% of the fat that goes away does so evenly. That appears to be one of the big strengths of my technique. The resorbed fat is carried away by your white blood cells in a natural way much like it would if you bruised your breast by blunt trauma.

Breasts after augmentation with fat grafting are usually incredibly soft and natural. It is usually impossible to detect the infiltrated fat even with careful examination. However, it is definitely possible to have calcifications and fat necrosis with fat grafting to the breast, as in every procedure to the breast. Silicone implants are especially prone to calcification and scarring. If fat necrosis occurs in a properly fat grafted breast it is usually isolated to a small area.

Please feel free to contact Renee at +1 212 571 5200 to ask further questions.

Hello Dr. Coleman,

I want to have this procedure done, but I dont want to have to gain weight and I dont want to have to give up exercising. Will I be able to exercise after having procedure done? Also, you mentioned somewhere about the effect of fat on the surrounding tissue. What does that mean? does the new fat change the consistency of the surrounding tissues? and what happens when the 20% of fat reabsorbs? Is it safe to reabsorb fat? also, I want the breasts to feel soft, but I heard calcifications and fat necrosis can make them hard?

RL,
With the current applicator, it would not work. Zeltiq will come out with more applicators in the future that will be usable on the face and neck.

K. Marks,
An interesting idea, but definitely not a current possibility. Perhaps something that might happen in the distant future, but not now.

Ella,
Use in the arm depends on the size of your arm, but it definitely could in a larger arm.
Zeltiq is in the process of developing new applicators that could be used smaller arms and other areas.
